Information on Scheduling an Appointment

To schedule an appointment with Dr. Levy, a doctor's referral is not required, but if your doctor's office is a rostered practice, you must call your doctor to schedule the appointment. Please note, the mechanism of the injury must be related to a sporting / fitness activity. We do not consult on injuries related to motor vehicle accident, or Workers' Safety Injury Claims at the Sports Medicine Clinic.

Physiotherapy appointments may be scheduled over the phone or in person, without a doctor's referral. We advise our patients to check with their supplemental health insurance company to determine if a referral from a physician is required for reimbursement of any physical therapy costs that may be billed directly to the patient.
